The Chiari network is movable, mesh-like structures oftenly present in the right atrium close to the opening of inferior vena cava and coronary sinus. This is mostly of no clinical importance and is often diagnosed incidentally. However, sometimes it can cause detectable confusion with right atrial pathologies, and may promote thromboembolism by causing flow obstruction. It may be related with infective endocarditis, arrhythmias, and migraine. At times, it acts as a physical hurdle during invasive procedures. The Chiari network has also been related to protect from pulmonary embolism by being an inferior vena cava filter due to its pore-bearing effect at the cavo-atrial junction. The Chiari network has been identified in a case of Ebstein anomaly of tricuspid valve which gave diagnostic confusion during echocardiography.
The Chiari network results from inability of resorption of the right sided sinus venosus valve. Evolutionarily, the right valve of the sinus venosus develops into two valves: the valve of the inferior vena cava i.e., Eustachian valve and the valve of the coronary sinus i.e., Thebesian valve. During involution of these valves, the tissue undergoes fenestration so that a network may be formed from remnants that usually disappears. The prevalence of Chiari network has variably been reported to be 2–13.6%.
